• Add life insurance - to take out a life insurance benefit if you already belong to SuperLife through your employer
• Add disability insurance - to take out a disability income protection insurance benefit if you already belong to SuperLife
• Add medical insurance - to take out a medical insurance cover benefit if you already belong to SuperLife through your employer

Update:

• Update personal details and/or beneficiary details - use this form to update:
  • Personal details - such as address, names and contact details
  • Beneficiary details - update the beneficiaries who will receive the benefits that are payable if you die

Change:

• Change life insurance - use this form to change your existing life insurance benefit if you are an individual Member
• Change disability income insurance - use this form to change your existing disability income protection insurance benefit if you are an individual Member
