• Investment statement and membership form: individual - to set up a superannuation scheme as an individual member
• Investment statement & membership form: employee - to set up a superannuation scheme as an employee member where all savings and insurances are voluntary i.e. there is no employer subsidy
• Investment statement & spouse membership form – insurance & savings - use this form to set up a spouce/partner for savings and/or insurance

Add/change:

• Change savings contributions - use this form to change your current monthly savings amount if you are an Individual Member
• Change investment strategy - use this form to change your investment strategy for your existing and/or your future savings
• Switch investments - use this form to switch some or all of your existing savings between different investment pools

Update:

• Update beneficiary details - use this form to update the beneficiaries who will receive the benefits that are payable if you die
• Update bank account - use this form to advise SuperLife of the bank account applicable for the payment of your benefits
