  • for 18 and older
  • for under 18 - to join children to SuperLife KiwiSaver, or to transfer their current KiwiSaver balance to SuperLife
• Direct debit form - to set up a direct debit payment to your SuperLife KiwiSaver account
• Lump sum payment form - to pay a lump sum to your KiwiSaver account

Change/update:

• Change investment option form - to change your investment option for your KiwiSaver account
• Contribution change form - non-employees - if you are not an employee and wish to change your savings level
• Contribution change form - employees - if you are an employee and you wish to change your savings level
• Update personal details - to update your details such as address, name and contact information
• PIR tax rate nomination - to advise SuperLife of your PIR rate

Apply:

• First Home withdrawal payment - to make a first home withdrawal from your KiwiSaver account
• Significant financial hardship application - to make an application for a payment under KiwiSaver for significant financial hardship 
• Permanent emigration withdrawal request - to make an application for a withdrawal payment if you are permanently emigrating from New Zealand.
• Serious illness withdrawal request - to make an application for a withdrawal payment under the KiwiSaver rules on serious ill-health
• Reinstatement form - to rejoin SuperLife or reinstate your membership of SuperLife 
• Contributions holiday - to go on a contributions holiday
