• Investment statement & membership form: individual - to set up a superannuation scheme as an individual member
• Investment statement & membership form: employee - to set up a superannuation scheme as an employee member where all savings and insurances are voluntary i.e. there is no employer subsidy
• Investment statement & spouse membership form – insurance & savings - use this form to set up a spouse/partner for savings and/or insurance
• Direct debit form - to set up a direct debit payment to your SuperLife accounts

Add/change:

• Add savings benefits - use this form to start saving if you are an Individual Member of SuperLife and already belong to SuperLife for Insurances
• Change savings contributions - use this form to change your current monthly savings amount if you are an Individual Member
• Change investment strategy - use this form to change your investment strategy for your existing and/or your future savings
• Switch investments - use this form to switch some or all of your existing savings between different investment pools
• Deposit or save a lump sum - use this form to save a lump sum if you already belong to SuperLife for savings

Update:

• Continuing SuperLife  - use this form to continue your membership when you change jobs if you joined through your employer
• Cancel membership - use this form to stop your life, disability or medical insurance benefits, or to stop your savings contributions
• PIR tax rate nomination - use this form to advise SuperLife of your PIR rate

Benefit request:

• Withdrawal payment request - use this form to request a payment of money from your superannuation (if you are a Member through your employer, payments will be made only if the rules of your employer's arrangements permit it)
• Managed income request - use this form to request a managed income benefit from your SuperLife savings accounts

Stop:

• Stop savings contributions - use this form to stop your current savings contributions
